Vitamin B12 is essential for red blood cell formation, DNA synthesis, and nervous system function. Unlike most vitamins, it is stored substantially in the liver — enough for 3-5 years. Metformin is the first-line diabetes medication and one of the safest drugs in medicine. However, it interferes with the calcium-dependent absorption of B12 in the small intestine. The effect is dose-related and duration-related:
The good news is that the mechanism is well understood, and calcium supplementation (or increasing dietary calcium) can partially restore B12 absorption. For most patients, however, B12 supplementation is the practical solution.
Key fact: B12-related nerve damage can mimic diabetic neuropathy almost exactly. Any diabetic with new or worsening numbness or tingling should have B12 checked before the symptoms are attributed to diabetes — treating the wrong cause wastes the reversible window.

⚠ Important: Neurological damage from prolonged severe B12 deficiency — balance problems, weakness, memory decline — can become permanent if untreated. Do not wait for classic anaemia; nerve symptoms often appear first, and blood counts can look normal despite significant deficiency.

Elevated MMA is a sensitive marker of tissue B12 deficiency. Used when serum B12 is borderline.

May show macrocytic anaemia (large red blood cells) or hypersegmented neutrophils — both suggestive of B12 deficiency.
Folate deficiency can mimic B12 deficiency and often coexists. Both should be tested together.
Modern evidence shows high-dose oral B12 (500-1,000 mcg daily) is effective for most causes of deficiency, including metformin-related. Convenience makes this the preferred first-line for most patients. Sublingual (under-the-tongue) preparations bypass some absorption issues.
Reserved for severe deficiency, neurological symptoms, malabsorption, or intolerance of oral forms. Standard protocol:
Both work. Methylcobalamin is the active form and may be preferred in neurological cases. Cyanocobalamin is more stable and widely available. For most patients, the difference is minor.
B-complex products combining B12, folate, and other B vitamins are common in India. Reasonable for maintenance in mild deficiency, though solitary B12 supplementation is usually sufficient.

Should I stop metformin if my B12 is low?
Usually no. Metformin’s benefits are substantial, and the deficiency is easily managed with supplementation. Stopping is considered only in unusual circumstances — decide with your diabetologist, never alone.
Are oral supplements really as good as injections?
For most causes — including metformin-related deficiency — high-dose oral B12 (500-1,000 mcg daily) restores levels effectively. Injections are reserved for severe deficiency, neurological symptoms, or true absorption failure like pernicious anaemia.
I’m vegetarian and my B12 is borderline. What should I do?
Start a daily oral supplement — dairy and eggs alone rarely maintain optimal levels, and Indian vegetarians on metformin carry double risk. Retest after 3 months to confirm response.
How quickly do symptoms improve with treatment?
Energy and blood counts improve within weeks. Nerve symptoms recover more slowly — over 3-12 months — and recovery is most complete when treatment starts early.
Can too much B12 be harmful?
B12 is water-soluble and excess is excreted; toxicity is essentially unknown at supplement doses. Very high blood levels usually just reflect supplementation.
Why test folate at the same time?
Folate deficiency causes similar anaemia and often coexists. Treating B12 while missing low folate — or vice versa — leaves the problem half-solved, and folate alone can mask B12’s blood signs while nerve damage continues.
Does B12 deficiency explain my fatigue even with normal haemoglobin?
It can. Neurological and energy symptoms frequently precede anaemia. A normal CBC does not exclude meaningful B12 deficiency — the serum level (with MMA if borderline) gives the answer.
