How variable red cell sizes are. A raised RDW with low MCV points toward iron deficiency; a normal RDW with low MCV is more suggestive of thalassaemia trait. This distinction matters in Kerala, where thalassaemia trait is not rare and iron supplements are often given unnecessarily.
Key fact: A raised MCV in a diabetic on long-term metformin should always prompt vitamin B12 testing — the CBC frequently spots this deficiency before any symptoms appear, while nerve damage is still fully reversible.
Normally about 4,000-11,000 per microlitre. Raised counts suggest infection, inflammation, steroid use, or stress; very high or very low counts need prompt evaluation.

⚠ Important: In a diabetic, anaemia distorts HbA1c results — sometimes substantially. Iron deficiency can falsely raise HbA1c, and correcting it can lower the reading without any change in actual glucose control. Always interpret HbA1c alongside a CBC.

Conversely, a single mildly out-of-range value in an otherwise well person often means very little. Reference ranges are set so that 5% of healthy people fall outside them by definition. Trends across repeated tests are far more informative than one flagged number.

Do I need to fast for a CBC?
No. Fasting is only needed if the CBC is bundled with glucose or lipid tests. Do stay well hydrated, though — dehydration falsely raises haemoglobin and haematocrit.

Why does my report show low haemoglobin but normal iron?
Several possibilities: anaemia of chronic disease, kidney-related anaemia, B12 or folate deficiency, or thalassaemia trait. This is exactly why MCV, RDW, and ferritin are needed — haemoglobin alone cannot identify the cause.
Can diabetes itself change my CBC?
Indirectly, yes. Diabetic kidney disease lowers haemoglobin, metformin can cause B12 deficiency raising MCV, and infections raise white cell counts. The CBC is genuinely informative in diabetes rather than a routine formality.
What is RDW and why does it matter?
It measures how variable your red cell sizes are. Small cells with high RDW point toward iron deficiency; small cells with normal RDW suggest thalassaemia trait. The distinction matters in Kerala, where unnecessary iron supplements are commonly given.
How often should I get a CBC?
Annually as part of a routine health check for most adults, and as part of annual diabetes review. More often if you have anaemia under treatment, kidney disease, or are on medications affecting blood counts.
