The Red Cell Values

Haemoglobin (Hb)

The oxygen-carrying protein and the headline number. Low haemoglobin means anaemia. Typical adult ranges are approximately 13-17 g/dL for men and 12-15 g/dL for women, with a lower threshold in pregnancy.

Haematocrit (PCV)

The proportion of blood volume occupied by red cells. It often tracks haemoglobin, but the commonly used “three-times-haemoglobin” rule is only an approximation and can fail in abnormal states. High values may reflect dehydration as well as true excess red cells.

Red Cell Count (RBC)

The number of red cells per volume. Interpreted alongside Hb and the indices below.

MCV — Mean Corpuscular Volume

  • Low MCV (microcytic): iron deficiency or thalassaemia trait — both very common in India
  • Normal MCV (normocytic): chronic disease, kidney disease, acute blood loss
  • High MCV (macrocytic): vitamin B12 or folate deficiency, thyroid disease, alcohol, certain medications

MCH and MCHC

The average haemoglobin content and concentration per cell. Low values accompany iron deficiency; they refine the MCV picture.

RDW — Red Cell Distribution Width

How variable red cell sizes are. A raised RDW with low MCV points toward iron deficiency; a normal RDW with low MCV is more suggestive of thalassaemia trait. This distinction matters in Kerala, where thalassaemia trait is not rare and iron supplements are often given unnecessarily.

Platelets

Normally 150,000-450,000 per microlitre. Platelets stop bleeding by forming clots.

  • High (thrombocytosis): iron deficiency, infection or inflammation, post-surgery, or, less commonly, a bone marrow disorder

⚠ Important: In a diabetic, anaemia distorts HbA1c results — sometimes substantially. Iron deficiency can falsely raise HbA1c, and correcting it can lower the reading without any change in actual glucose control. Always interpret HbA1c alongside a CBC.
